I would be cutting the umpire some slack for that. It was a freak occurrence, the ball hitting the off bail but not dislodging it. Umpire obviously heard the noise, so an understandable error rather than a howler, I'd say. Unfortunate for Rossington to be given out caught behind, but then again he was only saved from being bowled by a freak event.

Shouldn't the square leg umpire have picked this up?

Maybe so, in theory, but as I said I wouldn't be too critical of any umpire that missed it. The only time I've seen something similar, it needed DRS to clarify what had happened.
